Children with Medical Handicaps Program
The Population Health Department serves as the local contact for families who participate in this program. The CMH program provides early identification, evaluation, referral, and coordination of services for children from birth to 21 years with handicapping conditions. Under this program, the family is able to receive culturally competent home visits from the public health nurse which may include child assessment, care plan development, health promotion, and advocacy on behalf of the family.
There is no charge to families for participation in this program.
