GEAUGA PUBLIC HEALTH WANTS TO HEAR FROM YOU
Geauga Public Health (GPH), with University Hospitals, is currently conducting the 2025 Geauga County Community Health Needs Assessment (CHNA) and we want to hear from you, the public. This assessment is a way for GPH and University Hospitals to hear from the public about their health-related concerns like access to healthy food, smoking cigarettes, substance use, mental health challenges and other areas. Public engagement is key in this process, which is why folks from across Geauga County are invited to complete a community resident survey to let their voices be heard.
The community resident survey is now available for Geauga County residents 18 years of age and older and will cover a range of health-related topics. The survey will be available in both English and Spanish and can be accessed by visiting https://bit.ly/2025LGCHNA_English (English) and https://bit.ly/2025LGCHNA_Espanol (Spanish). The survey takes less than 15 minutes to complete and is both voluntary and anonymous. The survey will be available until at least Friday, January 31, 2025, and can be taken on your computer, tablet, or smart phone.
The purpose of the assessment is to understand the current health needs of Geauga County residents, as well as understand both where and how the health of the community can be improved. The assessment includes a review of national and state data sources, the community resident survey, and targeted community focus groups, which are currently happening across Geauga County.

Your voices, your experiences, and your insights are crucial to ensuring that this assessment truly reflects the needs and priorities of our community” said Administrator Adam Litke.
Hard copies of the English and Spanish language surveys will be available at Geauga Public Health, 12611 Ravenwood Drive, in Chardon, by request, and at select local partner buildings.
